Problem solved: 1- turn of the switches (this is the time that happens the problem); 2- save; 3- quit game; 4- continue; 5- switches worked! note: It may be that the first few times you do not work, but keep trying because it worked!

I didn't leave whole game just to the menu part and back into the world. Also I had to do it one more time at the end and the chest was on the ground. Hope it will help others too.

yeah i just had this bug as well the way i solved it waswhen u get close to the door that u can't open u should see the save icon in bottom left corner.The thing i did is - i read the order should be middle , right , left on one of the forums - I sent first one ghost to middle.When it got pulled i went close to the door and got a save, then i quit and enter again and the pulled switch was still pulled, so i did the same for right and left. Sent ghost, pulled, save. quit and enter again.

the bug trigger when there is one switch is pushed-in then when you want to use that switch with "interdiction", the solution is to pull out all the switchs and then solve the puzzle. to pull out that pushed switchs you must travel at the begining of the dungeon get to the puzzle and pull out the pushed switch, that will trigger the bug and then you must travel again to the begining, and again pull out another switch if there is another one pushed

i've got the same problem, i was looking for a solution please help, i don't want start again this game to meet the same one.
